Here's What You Need To Know:
1. How do I make an appointment?
  • There are a number of ways to schedule an appointment at the Arlington Heights clinic.  The easiest way is to click the "How Can We Help You Today?" chat button in the lower right corner and chat with one of our staff members, or send a Text Message 24 Hours a Day to 855-612-0411.  You can also schedule an appointment on our Facebook Page.  Pick the service you need and an appointment time and we will get right back to you to confirm.  You can always send us an email - MyHomeTownChiro@gmail.com, or call the office - 847.456.5409.

2. What happens after I schedule my first appointment?
  • You will receive an email from the office with all of the details regarding your appointment (date, time, our address, the cost, forms of payment) and the email will contain a link to our
  • NEW PATIENT HISTORY/CONSENT FORM for Chiropractor/Massage Therapy/Sauna/LED/HydroMassage visits, or
  • BLUE CROSS BLUE SHIELD PPO FORM for anyone who will be using Blue Cross Blue Shield PPO to pay for their visits.
  • AESTHETICS PATIENT CONSENT FORM for Aesthetics visits (Facials, Waxing, Lashes & Brows, Body Wraps, Microdermabrasion, etc.). or
     
  • Please fill out the form as soon as possible so we can get you set up in our patient database and the Doctor/Provider can review your history.

3. What is a VIP Membership?
​​
  • There are several types of VIP Memberships at HomeTown that are designed to save you the most amount of money on the type of services that you use most.  Wellness/Chiropractic Memberships, Massage Therapy Memberships, LED Light Therapy Memberships, Zerona Laser Fat Reduction Memberships, and Aesthetics Memberships.
  • VIP Wellness Members receive 2 free services every month (4 free services for Family Wellness Memberships) and substantial discounts on our Chiropractic/Wellness service fees by making a commitment to maintaining their health on a 3, 6 or 12 month basis.  In addition, a number of other benefits are awarded to our VIP Wellness Members that are designed to enhance their health, and that of their family members.  See the MEMBERSHIP PAGE for pricing and benefit information.
  • VIP Massage Members receive a 1-Hour Massage every month of this 12-month membership and additional massages at highly discounted rates.
  • VIP Zerona Laser Fat Reductions Members receive a Laser Session each month and a FREE LED Light Therapy or FREE Infrared Sauna session immediately following their Laser Treatment to maximize their results.
  • LED Light Therapy Members receive 8 LED Light Therapy Sessions each month to reduce the affects of aging and rejuvenate their skin.
  • Aesthetics Members get a free facial every month and permanent discounts on all Aesthetics Services in addition to extra discounts on our current sales including Facials, Waxing, Lash and Eyebrow Services, Body Wraps and more!

4. Do I have to become a member in order to be a HomeTown Health & Wellness patient?
  • Absolutely not.  We recommend membership to our patients that plan to come in once a month (or more), but our non-member patients receive the same wonderful care as everyone else.

5. What forms of payment can be used to pay for services?
  • HomeTown Health & Wellness accepts Cash, Checks, Debit/Credit Cards, HSA/FSA Cards, Apple Pay, Google Pay, Blue Cross Blue Shield PPO and Care Credit.  Keep in mind that there is a 3% transaction fee for all card transactions.

6. Does HomeTown Health & Wellness accept health insurance?
  • We accept Blue Cross Blue Shield PPO. Also some patients choose to send their HomeTown receipts into their insurance companies for reimbursement.

7. Does HomeTown Health & Wellness accept walk-in appointments?
  • The Arlington Heights office only accepts scheduled appointments, however, often times an appointment is available same day.  Tap the Chat Button in the lower right corner and ask if anything is available today.

8. What is the advantage to Membership?

9. I just want to do the Zerona Laser Fat Reduction, do I need to schedule a first visit with the Doctor before I can do Laser Treatment?
